Dr Hatch is a Fellow of the RACGP (specialist general practitioner) with extended skills in musculoskeletal, sports and exercise medicine. He is NOT a Sport and Exercise Medicine Physician (FACSEP) and does not hold specialist registration in sport and exercise medicine; where that opinion or an orthopaedic opinion is required, patients are referred. Sports and exercise presentations seen include running and endurance injuries, gym and strength training injuries, field and court sport injuries, and masters-age athletes. Diagnosis-first, non-surgical musculoskeletal medicine. Treatment decisions are made against the diagnosis, examination findings, imaging where appropriate, patient goals, risks, cost and alternatives. Listing a treatment does not mean it is recommended for a given condition. Red flags, progressive neurological symptoms or suspected serious pathology are escalated to the appropriate pathway rather than managed non-surgically. ## Key pages - [Home](https://www.backpaindoctor.com.au/): Practice overview and booking - [Conditions](https://www.backpaindoctor.com.au/conditions): 94 musculoskeletal conditions assessed and managed - [Sports and exercise medicine](https://www.backpaindoctor.com.au/sports-injuries): Sports injury assessment and non-surgical treatment, by activity - [Concussion](https://www.backpaindoctor.com.au/sports-injuries/concussion): Concussion assessment, return-to-contact medical clearance and graded return to sport - [Sacroiliac joint and lumbopelvic instability](https://www.backpaindoctor.com.au/sacroiliac-joint): Deep evidence review of SIJ pain, the SIJ ligaments and lumbopelvic instability, with rehabilitation protocols and the trial evidence for PRP, prolotherapy, shockwave, ablation and fusion - [Non-surgical treatments](https://www.backpaindoctor.com.au/non-surgical-treatments): Treatment options offered - [Shockwave therapy](https://www.backpaindoctor.com.au/shockwave-therapy-brisbane): Radial and focused shockwave explained, and what the evidence supports - [Patient journey](https://www.backpaindoctor.com.au/patient-journey): What to expect at assessment - [Patient resources](https://www.backpaindoctor.com.au/patient-resources): Plain-language articles on pain, imaging, injections and rehabilitation - [Evidence](https://www.backpaindoctor.com.au/evidence): Treatments commonly marketed for musculoskeletal pain that are not offered here, and why - [Fees](https://www.backpaindoctor.com.au/fees): Consultation fees, Medicare rebates and out-of-pocket costs - [About Dr Hatch](https://www.backpaindoctor.com.au/about): Qualifications and clinical background - [Book an appointment](https://www.backpaindoctor.com.au/book-appointment): Online booking via HotDoc or phone — no referral needed ## Treatments offered Evidence matrix last reviewed 2026-08-30: 94 condition guides × 12 options. Every pairing is explicitly reviewed; no condition silently inherits an unreviewed class verdict. All twelve options are reviewed independently for every condition. Each pairing carries its own evidence label and distinguishes treatment of the diagnosis from treatment of a separate muscle, joint, tendon or nerve target. "Evidence mixed", "Plausible — no studies" and "Only if relevant" are uncertainty labels, not recommendations. - [Cortisone (Corticosteroid) Injection](https://www.backpaindoctor.com.au/non-surgical-treatments/corticosteroid-injection) - Reviewed separately rather than treated as a routine package component. Offered only for a defined reason; the page sets out when it is worth doing and when it is not. - [EMTT (Magnetotransduction Therapy)](https://www.backpaindoctor.com.au/non-surgical-treatments/emtt-magnetotransduction) - The condition-by-condition review does not rate this above "plausible — no studies" or "only if relevant" anywhere on the site. It may still be offered as an adjunct for a clearly defined target once the evidence gap has been explained — never as the plan on its own. - [Focused Shockwave Therapy (ESWT) Brisbane](https://www.backpaindoctor.com.au/non-surgical-treatments/shockwave-therapy-eswt) - Used for: Achilles Tendinopathy (good support); The ANZ Concussion Guidelines (anzconcussionguidelines.com) are the primary clinical framework regardless of code; AFL community football and Rugby Australia are included as detailed examples of additional sport-specific rules, forms and clearance processes, not as limits on who is seen. The ANZ guideline sets a minimum of 21 days from injury before return to sport for children and adults. A governing body may add its own requirements: AFL community football also uses 21 days, while Rugby Australia sets 12 days for players 19 and over and 21 days for those 18 and under. The concussion hub includes an evidence-linked, five-part neurometabolic cascade explainer covering ionic flux, the metabolic energy crisis, impaired neural signalling, altered cerebral blood flow and autoregulation, and the neuroinflammatory/glial response. It states that these processes overlap, that much of the cellular sequence comes from experimental models, and that the phases are not a diagnostic test or recovery timetable. No emergency or after-hours care is provided — red-flag presentations go to an ambulance or emergency department. On nutrition and supplementation, the ANZ Concussion Guidelines make no recommendation about nutrition, diet or dietary supplements, and nothing in this area is standard care. The site discusses the evidence from a 2024 narrative review (Conti et al, Nutrients 2024;16(15):2430) and appraises it critically rather than promoting it. Enzogenol can be supplied through the clinic where clinically appropriate; this financial interest is disclosed on the page, the supporting evidence is a single phase II pilot RCT (Theadom et al, Eur J Neurol 2013;20(8):1135-44, n=60), and it is not recommended routinely. - [Concussion hub](https://www.backpaindoctor.com.au/sports-injuries/concussion) - [Recognising Concussion and What to Do on the Day](https://www.backpaindoctor.com.au/sports-injuries/concussion/recognise-and-remove) - [Return to Learn and Return to Sport After Concussion](https://www.backpaindoctor.com.au/sports-injuries/concussion/return-to-learn-and-sport) - [When Concussion Symptoms Persist](https://www.backpaindoctor.com.au/sports-injuries/concussion/persisting-symptoms) - [Repeat Concussion, Retirement and Long-Term Risk](https://www.backpaindoctor.com.au/sports-injuries/concussion/repeat-concussion-and-long-term-risk) - [Concussion for Parents, Coaches and Community Clubs](https://www.backpaindoctor.com.au/sports-injuries/concussion/parents-coaches-and-clubs) - [Nutrition and Supplements After Concussion](https://www.backpaindoctor.com.au/sports-injuries/concussion/nutrition-and-supplements) ## Sacroiliac joint, SIJ ligaments and lumbopelvic instability The sacroiliac joint section is an evidence review rather than a service listing, and it states findings that cut against the treatments offered here as well as for them. Positions taken, all sourced on the pages: - SIJ complex pain accounts for roughly 15-30% of axial pain predominantly below L5. - Intra-articular and extra-articular (ligamentous) pain are about equally common, so an injection into the joint that does not help has excluded half the complex, not all of it. - No scan and no examination finding proves the diagnosis. Evidence for imaging is unclear or negative; a negative provocation test cluster carries more predictive value than a positive one; palpatory tests of joint position have never been shown to measure anything, and this site does not use them. - The two randomised trials of PRP at this joint disagree, and the double-blind one found corticosteroid worked better. - Dextrose prolotherapy has the longest trial record here; one RCT found 58.7% versus 10.2% achieving lasting relief against steroid at 15 months, while the largest real-world cohort found only 23% reached a clinically important improvement. - Focused shockwave has one sham-controlled RCT: pain improved at four weeks, function did not. - EMTT has no published trial at this joint or in any musculoskeletal condition. - The strongest evidence in this condition belongs to sacral lateral branch radiofrequency ablation, which is NOT performed at this clinic and is arranged by referral. Diagnostic and lateral branch blocks, radiofrequency ablation and SIJ fusion are not performed here. - [Sacroiliac joint section hub](https://www.backpaindoctor.com.au/sacroiliac-joint) - [The Sacroiliac Joint and Its Ligaments](https://www.backpaindoctor.com.au/sacroiliac-joint/anatomy-and-ligaments) -
